Book Report Form

Book Report Form
Title: _____________________________________________________________
Author: ___________________________________________________________

  1. What is the setting of the book?  Tell the place and time.
  2. Who are the main characters?  Describe at least 5 using at least a sentence per character.
  3. Describe two connections you made to the story.  For example, a similar experience that you had, how a character is similar to someone you know, or how an event in the story is similar to another book or news event.  Write at least 4 sentences per connection.
  4. If a main character were removed from the book, how would the book change and why? "There would be no story" is not an appropriate response.   Write at least 4 sentences.
  5. Take a minor character from the book that you would like to see play a larger role. Write at least 4 sentences in which your character interacts with one of the main characters; stay true to the novel.
  6. If you were a major character in the book, how would you have responded to the conflict and why?  Write at least 4 sentences.
  7. What was your favorite part of the story?  Why?  What was your least favorite part and why?  What would you change about the book if you were the writer? "I wouldn’t change anything" is not an appropriate response.
  8. Fill out the chart on the back, using at least 15 different events.  Remember, the conflict is the main problem, the rising action is how the plot builds up, the climax is the high point of the action, and the resolution is how the problem is solved and they all live happily (or unhappily) ever after.
